Well, I got all excited and thought this was the magic bullet. On the next hotspot I tried (4.0.0-RC4), I put it into read-write mode and then the update ran fine, though it finished in read-write mode (that long running problem). Then I rebooted and tried running an update without putting it in read-write mode first and the Updating DV Binaries step failed. Aha! I thought. Then I rebooted again, put it into read-write mode, and tried running another update.... Alas, the Updating DV Binaries step failed again. Argh! Back to the drawing board.

Examining /usr/local/sbin/pistar-update, it's easy to see that the code for "rpi-rw" is invoked, since the script is run as root:

To answer KE7FNS' question, if you'll run:

cat /proc/sys/net/ipv6/conf/all/disable_ipv6
we'll be able to see if you have IPv6 enabled or not. The command will return a zero "0" if IPv6 is enabled, a one "1" if IPv6 is disabled.

Apologies guys, I was wrong. I ran

cat /proc/sys/net/ipv6/conf/all/disable_ipv6
and it returned zero, so it appears ipv6 is enabled on my system.

I looked in my VPN settings and verified that it was off there, as I remembered. Then I looked at Airport Utility (my router is an Airport Extreme) and there I saw that ipv6 is enabled. I had never seen that before, but I got a new Mac Mini recently, so I guess it came with a new version ofAirport Utility that enables ipv6 by default.

I took a look at Apple's communities and found a question asking how to disable ipv6 on newest airport utility. The answer was: You cannot disable IPv6, but you can disable the IPv6 Internet connection coming from the Internet by setting IPv6 Mode to Link Local Only. I did that, then rebooted, and rebooted my hotspot, too, but the above command still returns zero.

I'm at a loss for what to do next. Any ideas?

One can disable IPv6 on a running system with (as root):

echo 1 > /proc/sys/net/ipv6/conf/all/disable_ipv6
It isn't enabled by default in v3.4.17:

grep disable_ipv6 /etc/sysctl.conf
returns:
net.ipv6.conf.all.disable_ipv6 = 1

If KE7FNS thinks that it might be an issue for you, then you might want to replace the "0" with a "1". It seems to be enabled with "0" by default in the Pi-Star v4-rc3 image at least, which explains why your reboots always enable it.

If disabling IPv6 solves your problem then you might consider a "Feature Request" to present an IPv6 option to disable on the Configuration page.

Weird.

The other day I noticed an IPV6 address on my login page of pi-star, I didn't think much about it, but now it seems to be disabled according to that command posted above, and I never disabled or enabled it.

It was just a hunch that it could of been causing an issue, I can't even explain how I came up with the notion in the first place.

Good news: it looks like the 20190521 update may have partially fixed this issue. So far this morning, I've tried three different models of radio/modem boards and have had the same experience with all three: the first update I run (from 20190519 to 20190521) gets hung on Updating DV Binaries. But after that, subsequent updates, whether run through the dashboard or a separate SSH app, run just fine and don't get hung. Fingers crossed, but the following commits that were checked in a couple hours ago may have fixed this issue. I will be so happy if they did!
