Not heared at all

My Pi-star is connected to DCS010B, I can se my call on the reflector, I hear other stations and I hear can my own echo
when trying. The log in Pi-star shows no error when transmitting my TH-D74. The signal is 59 +46dB. What am I missing?

Not many details in your report, but here's the very basics: For most simplex hotspots, you must use D-STAR Repeater (DR) or Duplex mode: set up RPT1, RPT2, and a zero offset (either +/−0.000).

Ok, sorry. I should have mentioned my settings.
RPT1: SM7RDX B
RPT2: SM7RDX G
controller mode: simplex node
no offset (00000)
reflector DCS010B
coordinates not important.

TH-D74-settings:
DR mode
00000 offset
I have tried + - and no shift
UR: CQCQCQ
linked reflector DCS010B
set USE REFLECTOR
I have chosen my hotspot in repeaterlist

and ofcourse the same qrg on both, they are talking to each other.

Have I forgotten something?

Two things you might check:

1. When I set my URCALL field on my TH-D74A, I do it via Destination Select > Direct Input (URCALL). I know that it will not work if the UR CALL is set to CQCQCQ using the "Local CQ" option.

2. A gotcha to be aware of: Direct Reply

There's a D-STAR feature on the TH-D74A called Direct Reply. When a call has been received, the green Interrupt screen is displayed for a few moments. If Direct Reply is enabled, as long as the green Interrupt screen is displayed, you can initiate a direct reply to the caller by pushing PTT. However, this doesn't work if you're using a hotspot.
If you're using a hotspot and reply using Direct Reply, your transmission won't be heard. There are two ways to get around this: turn off Direct Reply (menu 612), or if you want to leave the feature enabled, press the [ A/B ] = OK button to close the Interrupt screen before pressing PTT.

I have read https://amateurradionotes.com/th-d74.htm many times and there is a
link to a Youtube clip made by W6GPS. I noticed that at the bottom of his display
it says UR: CQCQCQ but in my display it says UR: SM7RDX
I have only succeded to have CQCQCQ if I disconnect from reflector or change to DV mode.
Neither is good.
When I link to the reflector a voice says "linked to...",the display says UR: CQCQCQ and at the bottom
I read the name of the reflector. Looks good! But when I press ptt "UR: CQCQCQ" disapears
and my call sign returns.
Back at square 1.
I have followed every step in the article and it shouldn't be any difference
between D74A and D74E other than qrg.
I have started to think it might be something with the firmware, I have 1.09
but it can't be...

There are som clips to see on http://downtownsmedby.se/dstar mostly in swedish but the first part is not.

That's definitely a problem. I've never seen that happen on my TH-D74A (also on 1.09). The only final suggestion I can make is to go over your CPS entries again very carefully; it's possible there is some entry that is not quite right.

Do you mean on the GREEN screen phase? Because the clip you show at http://downtownsmedby.se/dstar/UR.mp4 appears to behave identical to my unit (it shows my callsign at the conclusion of transmission, and shows CQCQCQ when the other end transmits -- ie: during the "linked"/"not linked" announcement or when receiving someone else).

For example, using the "reflector information" destination

<green screen> shows CQCQCQ and hotspot voice announcement sounds.

SM7RDX isn't a registered D-Star call sign with the U.S. Trust Server ... I'm not sure how that works in Sweden or with DCS reflectors but here it's https://dstargateway.org/D-Star_Registration.html for your consideration.

In order to be able to use the D-STAR gateway on the repeater, each HAM operator must apply for an approval within the US Trust Server network, which is done at the registration webpage at any US Trust Server connected gateway repeater.
So that could be the most viable problem the OP has -- local configuration may be working properly, but lack of registration means things are just going into a bit-bucket...
