Your message indicates you did an UPGRADE not an UPDATE. You must run updates before running an upgrade. You may need to run updates multiple times, until it comes up clean (no further updates) before running an upgrade. And if you're on 4.1.2, you'll then need to run the upgrade multiple times until it says you are at the current level.
