MMDVM needs HDMI display connected

If I have the HDMI display connected, pi-star boots and runs for days. Soon after disconnecting the display, it locks up and nothing shows on the OLED display.

Shouldn't be a factor and just a coincidence as nothing in pistar is reliant of having an active screen connection.

Everything is built to run as a stand alone server type instance.

I had pi-star running for 3 days. After I disconnected the HDMI, it shutdown/locked up within minutes.
Reconnected HDMI and rebooted OK.

Pi's in general don't handle voltage sags/surges very well, particularly with high demand devices like monitors, particularly with earlier models of Pi's, so indiscriminate plugging/unplugging can be disruptive. And, out of the box, Pi-Star does not come with HDMI hot-plugged. You can try adding that option (hdmi_force_hotplug=1) to config.txt but it may or may not help in your current hardware mix.
